They are In Ear Monitors (as opposed to monitors as speakers on the stage floor pointed at the performer) and they are commonly custom-molded to completely fill the outer ear. The performer can typically create their own mix with levels of the other performers and their own adjusted to their preference. The fan would have to be speaking into a microphone that was in that mix to be heard by the performer.
